Facilities and software include a broadband internet connection, separate business phone and fax line, laser printer, Microsoft Word, PowerPoint, Excel and EndNote bibliographic/database search software.
I generally prefer to work based on an hourly rate, with an estimate if required, but sometimes work on a quote/project fee basis.
